Job Summary
Provide case management, coordination, supervision, and implementation of professional and supportive services to clients using nursing theory and process in accordance with the Nurse Practice Act. Review client referral information and respond to requests and inquiries as appropriate. Perform client assessments based on agency standards of practice to ensure effective and appropriate home care services. Direct, plan, and initiate appropriate action independently and responsively in-home care situations. Perform job duties in accordance with agency policies, procedures, and professional and community standards. Perform other job duties as assigned by the Director of Clinical Services. This full-time role in Montrose, CO, includes Monday through Friday 8-hour shifts with occasional weekends, offering a supportive team environment for experienced nurses providing individualized health care to older adults across Delta, Montrose, and Ouray counties.
Required Qualifications
- Current license as a RN in the state(s) of Colorado
- Minimum of two (2) years of nursing experience, with a combination of long-term care dementia care or acute care
- CPR certified
- Graduate of an accredited school of professional nursing program

- Baccalaureate degree preferred
- Home care experience preferred
