Registered Nurse - RN

Registered Nurse in Home Therapies program provides patient education and training for home dialysis (peritoneal and hemodialysis), coordinates clinical care, and supports patients and families. Responsibilities include assessing patient learning needs, developing and updating education plans, directing care with home dialysis patients and Home Partners, administering medications per algorithm, initiating emergency response measures, collaborating with the interdisciplinary team, documenting in medical records, and participating in CAP requirements for advancement. The role may involve on-call rotation, travel to training sites or patient homes, and supervision of staff as a Team Leader or RN in charge when assigned.
