Job Summary

Conduct one-on-one training and education for dialysis patients on peritoneal and home hemodialysis using state-of-the-art equipment. Perform initial home visits with another staff member present while functioning as an onsite Patient Educator and Case Manager. Assess, plan, and evaluate care for assigned patients by following prescribed protocols, administering medications, and initiating emergency response measures. Collaborate with the interdisciplinary team to document outcomes, update care plans, and ensure correct laboratory collection procedures. Train home partners on safe equipment operation and maintenance while identifying barriers to home environment readiness. Complete CAP requirements to advance, rotate coverage with other licensed staff, and participate in on-call rotations including nights, weekends, and holidays.

Required Qualifications

  • Current appropriate state licensure
  • Current or successful completion of CPR BLS Certification
  • Must meet the practice requirements in the state in which he or she is employed
  • Must have a minimum of 12 months experience as a RN
  • Successful completion of all FKC education and training requirements for new employees
  • Must be able to lift 30 lbs as high as 5 feet
  • Must be able to move machines and equipment of up to 200 lbs with assistance
  • Must be able to bend over
  • Must be able to stand for prolonged periods
  • Must be able to move with assistance
  • Must be available for on-call rotation
  • Must be available for night shifts
  • Must be available for weekend shifts
  • Must be available for holiday shifts
  • Must be available for shifts as defined by individual program needs
  • Must be able to travel to training sites, other facilities or patient homes
  • Must be able to work in a health care facility with moderate noise levels
  • Must be able to be exposed to infectious and contagious diseases/materials
  • Must be able to perform essential functions in other locations including patient's home

Desired Qualifications

  • Minimum of 1-year experience as a Registered Nurse (preferred)
  • Home dialysis therapy experience (preferred)
  • Successful completion of 3 months experience as a RN in home peritoneal dialysis and / or hemodialysis
  • Entry level for RNs with less than 2 years of Nephrology Nursing experience as a Registered Nurse
