Job Summary
Conduct one-on-one training and education for dialysis patients on peritoneal and home hemodialysis using state-of-the-art equipment, primarily working in the clinic setting while conducting initial home visits with another staff member present. Assess and manage patients' response to home dialysis therapy by following prescribed protocols, administer medications as prescribed, and document all interventions in the patient medical record. Collaborate with the interdisciplinary team to identify barriers to home dialysis readiness, evaluate home environments, and train patients and home partners on safe equipment operation and maintenance. Perform ongoing data collection and analysis for assigned patients, adjust treatment plans as indicated, and communicate patient issues to physicians as needed.
Required Qualifications
- Current appropriate state licensure
- Current or successful completion of CPR BLS Certification
- Must meet the practice requirements in the state in which he or she is employed
- Must have a minimum of 12 months experience as a RN
- Successful completion of all FKC education and training requirements for new employees
- Must be able to lift 30 lbs as high as 5 feet
- Must be able to move machines and equipment of up to 200 lbs with assistance
- Must be able to bend over
- Must be able to stand for prolonged periods
- Must be able to work night, weekend, holiday, or as defined by individual program needs
- Must be able to rotate coverage with other licensed home therapies staff
- Must be able to travel to training sites, other facilities or patient homes
- Must be able to work out of the Delaware program
- Must be able to work out of Marion, OH as needed
- Must be able to work Monday - Friday 8am- 4:30 hours (flexed depending on patient needs)
- Must be able to work in a health care facility with moderate noise levels
- Must be able to be exposed to infectious and contagious diseases/materials
- Must be able to perform essential functions of the position in other locations including patient's home
Desired Qualifications
- Entry level for RNs with less than 2 years of Nephrology Nursing experience as a Registered Nurse
- Minimum of 1-year experience as a Registered Nurse (preferred)
- Home dialysis therapy experience (preferred)
- Successful completion of 3 months experience as a RN in home peritoneal dialysis and / or hemodialysis
