Registered Nurse (RN)

Job Summary
Provide in-home nursing care to patients in the West Lafayette, IN area under physician-ordered plans of care; perform patient assessments and ongoing evaluations; complete required documentation (OASIS) and coordinate services while educating patients and caregivers on medications and disease processes. Supervise LPNs/HHAs as needed, communicate changes in patient condition to physicians and care teams, maintain compliance with regulations and agency policies, and utilize a tablet for documentation and training.
Required Qualifications
- Graduate of an accredited school of professional nursing (required)
- Current Registered Nurse license in the state of Indiana (required)
- Minimum of 1 year nursing experience in a hospital, facility, or home health setting; home health experience (preferred)
- Valid driver’s license, reliable auto insurance, and personal transportation (required)
- Current up to date TB test
