Registered Nurse (RN)

Job Summary
Provide in-home nursing care to multiple patients per day by performing patient assessments, implementing physician-ordered plans of care (including OASIS), and completing required documentation. Educate patients and caregivers on medications and disease processes, coordinate patient services, and supervise LPNs/HHAs as needed. Communicate changes in patient condition to physicians and care teams, and maintain regulatory and agency-compliance standards. Requires driving for in-home visits and use of a tablet for documentation and training.
Required Qualifications
- Graduate of an accredited school of professional nursing
- Current Registered Nurse license for practicing state
- Minimum of 1 year nursing experience in a hospital, facility, or home health setting; home health experience (preferred)
- Valid driver’s license, reliable auto insurance, and personal transportation
- Current up to date TB test
