Job Summary
Conduct comprehensive assessments of patients and families to determine home care needs, including physical assessments and histories of current and previous illnesses. Initiate and revise plans of care with physicians, establishing goals based on nursing diagnoses while incorporating therapeutic, preventive, and rehabilitative actions. Administer medications and treatments as prescribed, counsel families on nursing needs, and provide health care instructions per assessment. Perform high-tech nursing procedures such as catheter insertion, ostomy care, and negative pressure wound therapy while observing infection control and ethical guidelines. Assess physical status through health data and interviews, re-evaluate needs, and inform physicians of condition changes. Prepare clinical notes, update the primary physician every sixty days, and coordinate care among home, community providers, and pediatric specialists. Supervise home health aides and LPNs/LVNs per regulatory stipulations. Participate in on-call duties and ensure availability of necessary equipment and supplies.
Required Qualifications
- Bachelor's degree of Nursing from an accredited college/university
- Current active and unrestricted nursing license
- Valid driver's license
- Current certification in Basic Life Support for Health Care Professionals
- Minimum of 1 year of recent acute or home care experience, preferably in an institutional setting
- Excellent observation, verbal and written communication skills, problem solving skills, basic math skills, nursing skills per competency checklist
- Basic knowledge of computer operating systems and software applications to communicate and document care
- Prior experience working with the Adults/ Geriatric population
- Prior experience working with the Special Needs population
- Pediatric experience
- Experience in pediatric nursing from birth through late teens
- Experience with community health education
- Knowledge of high-tech nursing skills as applicable which is not limited to indwelling/straight catheter insertion/removal, nephrostomy care, ostomy care, plural and abdominal catheter drainage care, infusion services, g-tubes, and negative pressure wound therapy
- Working knowledge of all infection control procedures and safety precautions
- Knowledge of the nurse's scope of practice and ability to apply professional theory, practice and procedure showing appropriate application of the nursing process
- Ability to observe ethical and legal guidelines for safeguarding confidentiality of patients
- Ability to assess patient needs and physical status each skilled visit through health data assessment and patient interview
- Ability to re-evaluate patient needs through physical assessment, response to therapy and any updated physician's orders
- Ability to inform physician of any changes in patient condition, response to care, progress etc
- Ability to obtain and clarify orders for Plan of Treatment revisions
- Ability to communicate with the physician regarding the patient's needs and reports and changes in the patient's condition
- Ability to obtain/receive physician's orders as required
- Ability to communicate with other health related persons to coordinate care plan
- Ability to participate in on-call duties as defined by the on-call policy
- Ability to ensure that arrangements for equipment and other necessary supplies and services are available
- Ability to instruct, supervise and evaluate home health aide/personal care aide care provided every 14 days or as stipulated by law and regulation
- Ability to instruct, supervise and evaluate LPN/LVN care provided at least every 30-days
- Ability to exercise professional management
- Ability to coordinate care between and among involved parties in home, community providers and pediatric tertiary specialists
