Job Summary
Conduct in-home healthcare visits and comprehensive geriatric assessments for clients in Lake and Porter Counties. Complete nursing assessments, monitor client conditions, and develop individualized care plans while coordinating home health services and community resources. Communicate with families, physicians, and care teams to advocate for client safety and independence. Maintain accurate clinical documentation and ensure compliance with all applicable regulations and professional standards. This dual-employment role balances responsibilities across Firefly Home Health and First Call Care Management for a flexible part-time or PRN schedule.
Required Qualifications
- Bachelor's Degree
- Active and unrestricted Indiana Registered Nurse (RN) license
- Registered Nurse applicants only (LPN applicants will not be considered)
- Minimum of two years of experience in home health, geriatric care, case management, or a related healthcare setting
- Strong understanding of healthcare systems and resources available to older adults
- Excellent assessment, communication, documentation, and care-planning skills
- Strong organizational skills with the ability to manage multiple responsibilities
- Ability to work independently while collaborating effectively with families and healthcare professionals
- Valid driver's license and reliable transportation
- Ability to travel throughout Lake and Porter Counties
- Compassionate, dependable, patient, and professional demeanor
Desired Qualifications
- Geriatric Care Manager or Aging Life Care Manager certification preferred
