Job Summary
Conduct comprehensive nursing assessments, admissions, supervisory visits, and discharge visits for clients in their homes while developing individualized care plans. Perform skilled nursing services, educate members and caregivers on disease management and safety, and document all visits accurately in Home Care BOSS and electronic systems. Communicate effectively with physicians, case managers, and families, reporting significant condition changes immediately. Maintain HIPAA compliance, follow infection prevention standards, and participate in quality assurance activities. Full-time, Monday through Friday day shift with rotating on-call availability. Requires active Georgia RN license, minimum one year experience, and valid driver's license.
Required Qualifications
- Active Georgia Registered Nurse (RN) license in good standing
- Minimum one (1) year of nursing experience
- Current BLS Certification
- Valid Georgia driver's license
- Reliable transportation
- Current automobile insurance
- Ability to pass background screening
- Strong assessment and documentation skills
- Excellent communication and organizational skills
- Ability to work independently in the field
- Ability to travel throughout assigned service areas
- Ability to lift up to 25 pounds
- Ability to stand, bend, kneel, and walk for extended periods
- Ability to safely enter and exit members' homes
- Knowledge of HIPAA regulations
- Dependability and accountability

- Home health experience
- Medicaid Waiver experience (CCSP, SOURCE, ICWP)
- Home Care BOSS experience
- Electronic Medical Record (EMR) experience
