Job Summary
Provide in-home nursing care to patients of all ages across a designated area, using assessment tools to identify individualized needs and executing care plans. Build rapport with patients, maintain and update professional records, and travel to patient residences for visits. The role includes on-call coverage after hours and the opportunity to work part-time or full-time with weekend rotation, supported by a team and comprehensive training. The position emphasizes autonomy, independence, collaborative care, and long-term therapeutic relationships, with a mission to deliver compassionate, person-centered home health services and enable patients to remain at home safely.
Required Qualifications
- Registration with the CNO as a Registered Nurse in good standing
- Valid driver’s license and access to a vehicle
- Experience in delivering community care is an asset
- Willingness to travel to and from patient residences within a designated area
- On-call coverage, including after hours to ensure patient safety and care needs
