Registered Nurse (RN)- Discharge Care, Part-Time, Remote, Weekends

Provide non–face-to-face post-discharge outreach calls to discharged patients, conduct medication reconciliation and coordination support, assess patient understanding of discharge plan and adherence, document interactions in the EHR, escalate issues to the provider or care team, schedule appointments with the patient’s primary care provider, and contribute to readmission-prevention efforts through timely follow-up and addressing barriers to care. Collaborate with providers, care coordinators, and clinical teams; educate patients on discharge instructions and when to contact a provider; perform nursing observations and ensure compliance with CMS requirements and organizational standards.
