Job Summary
Registered Nurse - Home Therapies (Dialysis) provides one-on-one patient education and coordinates home dialysis training (peritoneal and home hemodialysis) in clinic settings and during initial home visits with another staff member. Functions as an onsite Patient Educator and Case Manager, assessing and planning individualized nursing care, delivering dialysis treatment, training patients and Home Partners on equipment and protocols, and participating in the End Stage Kidney Disease health care team. Role includes teaching, leadership activities, and involvement in quality improvement, with CAP entry-level advancement and requirement to complete CAP education. Responsibilities include data collection and documentation, education delivery, appointment coordination, emergency response, and collaboration with the Interdisciplinary Team to optimally manage home dialysis patients and home environment readiness.
Required Qualifications
- Graduate of an accredited School of Nursing
- Current appropriate state licensure
- Current or successful CPR BLS Certification
- Experience: Entry level for RNs with less than 2 years of Nephrology Nursing experience; Minimum 1-year RN experience (preferred); Home dialysis therapy experience (preferred)
