Job Summary
Registered Nurse - Dialysis educator and case manager in a clinic/home-therapy setting. Responsible for training dialysis patients on peritoneal and home hemodialysis, providing initial home visits with a second staff member, and coordinating clinically competent care under physician direction. Role includes ongoing patient education, data collection and documentation, collaboration with Home Dialysis teams, administering prescribed medications, participating in emergency response, guiding Home Dialysis candidates and Home Partners, and supporting CAP education and advancement. Requires graduation from an accredited nursing program, active state RN licensure, CPR/BLS certification, and at least 12 months RN experience (nephrology nursing experience preferred). The position entails clinical duties, patient education, team leadership, on-call coverage, and travel to training sites or patient homes as needed.
Required Qualifications
- Graduate of an accredited School of Nursing
- Current appropriate state licensure
- Current or successful completion of CPR BLS Certification
- Must meet the practice requirements in the state in which he or she is employed
- Minimum of 12 months experience as a RN
- Entry level for RNs with less than 2 years of Nephrology Nursing experience (preferred)
