Job Summary
Provide and coordinate clinically competent care for Home Dialysis patients as part of the End Stage Kidney Disease (ESKD) care team. Assess and manage patient responses to home dialysis training and treatment per predefined protocols; document data in the patient record; educate patients and families; train Home Dialysis patients and Home Partners on equipment and treatment; participate in CAP activities, quality improvement, and staff training. Responsibilities include administering medications per approved algorithms, responding to emergencies, coordinating with the interdisciplinary team, ensuring laboratory procedures are performed correctly, and supporting transplant/treatment modality discussions. Must be a graduate of an accredited School of Nursing with current state licensure, CPR/BLS certification, and a minimum of 12 months RN experience (nephrology experience preferred). Position involves on-call rotation, potential travel to training sites or patient homes, and collaboration with supervisors, Educator, and preceptors to ensure safe and effective dialysis care.
Required Qualifications
- Graduate of an accredited School of Nursing
- Current appropriate state licensure
- Current CPR/BLS Certification
- Successful completion of CAP requirements to advance
- Minimum 12 months experience as an RN
- Home Dialysis experience (preferred)
