Job Summary

Assess, plan, and coordinate clinically competent dialysis care for assigned patients, including delegation to direct care staff and collaboration with the interdisciplinary team. Perform ongoing data collection and analysis to adjust treatment plans, administer medications per protocol, and document medical justification. Provide initial and ongoing education to patients and families regarding transplant options, treatment modalities, and home dialysis equipment operation. Mentor new and incumbent employees, serve as a preceptor, and lead quality improvement initiatives while rotating coverage with home therapy staff. Requires night, weekend, and holiday on-call rotation with travel to training sites and patient homes.

Required Qualifications

  • Registered Nurse (RN)
  • Graduate of an accredited School of Nursing
  • Current appropriate state licensure
  • Current or successful completion of CPR BLS Certification
  • Must meet the practice requirements in the state in which he or she is employed
  • Minimum of 2 years or more of Nephrology Nursing experience within the last 2 years as a RN
  • Must have a minimum of 3 months experience home dialysis therapies as a RN
  • Must be able to lift chemical and water solutions of up to 30 lbs.
  • Must be able to move, with assistance, machines and equipment of up to 200 lbs.
  • Must be able to bend over
  • Must be able to stand for frequent, prolonged periods
  • Must be able to lift and move patients and assist with ambulation
  • Must be available for on-call rotation
  • Must be available for night shifts
  • Must be available for weekend shifts
  • Must be available for holiday shifts
  • Must be available for travel to training sites, other facilities and patient homes
  • Must be willing to provide essential functions of this position in other locations
  • Must be able to perform dialysis therapies
  • Must be able to administer medications as prescribed
  • Must be able to perform laboratory collection, processing and shipping procedures
  • Must be able to perform assessment and identify barriers of the Home Dialysis candidate's home environment
  • Must be able to train home dialysis patients and / or Home Partners on the safe, effective operation and maintenance of all Home Dialysis equipment and treatment supplies
  • Must be able to participate in education and quality improvement projects
  • Must be able to serve as a Preceptor to new employees
  • Must be able to delegate appropriate tasks to direct patient care staff including but not limited to RNs, LVN/LPNs and Patient Care Technicians
  • Must be able to participate in decision-making, teaching, leadership functions, and quality improvement activities
  • Must be able to ensure correct laboratory collection, processing and shipping procedures are performed
  • Must be able to reschedule missed or insufficient laboratory collections
  • Must be able to identify expected outcomes, documents and updates the nursing assessment and plan of care for assigned patients
  • Must be able to ensure patient awareness related to transplant and treatment modality options
  • Must be able to assist in the identification, evaluation, selection and education of Home Dialysis candidates and Home Partners
  • Must be able to assess, collaborates and documents patient/family's basic learning needs to provide initial and ongoing education to patients and family
  • Must be able to direct and provide, in collaboration with the patient, home care partner, direct and ancillary patient care staff, all aspects of the provision of safe and effective delivery of dialysis therapies to assigned patients
  • Must be able to perform ongoing, systematic collection and analysis of dialysis data for assigned patients
  • Must be able to makes adjustments or modifications to treatment plan as indicated
  • Must be able to notifies Supervisor or physician as needed
  • Must be able to initiates or assists with emergency response measures
  • Must be able to serves as a resource, leader, coach, mentor and role model for new and incumbent employees
  • Must be able to set examples of appropriate behavior, work habits and attitudes towards patients, co-workers, supervisors and the company at the facility and area level
  • Must be able to perform all essential functions under the direction of the Supervisor and with guidance from the Educator, Preceptor or in collaboration with another Registered Nurse
  • Must be able to perform all duties as assigned by Supervisor
  • Must be able to complete CAP requirements to either maintain or advance
  • Must be able to perform all duties in accordance with Fresenius Kidney Care policy, procedures, standards of nursing practice, state and federal regulations
