Job Summary
Provides care and treatment to dialysis patients as part of a multidisciplinary team. Performs and maintains proficiency in dialysis techniques, oversees and delegates to department staff, ensures compliance with water room equipment operation, safety, infection control, and regulatory standards (CMS/ESRD, QA/PI). Documents in patient records per policy, coordinates care with Medical Director, nephrologists, patients and families, and other nursing staff. Develops and implements patient and caregiver education plans related to dialysis processes and self-care, and supports care coordination and advocacy to optimize patient outcomes.
Required Qualifications
- Registered Nurse (RN) license in the state of practice
- Active RN license
- Competency in dialysis-related care
- Ability to meet essential job functions including visual/hearing requirements
- Home dialysis nursing experience preferred or as described in the role
