Registered Nurse - RN

Provide and direct nursing care to patients in their homes as prescribed by physicians and agency policies. Coordinate total plans of care with health care professionals, physicians, and community resources to ensure continuity. Perform comprehensive nursing evaluations, determine primary care focuses, and develop plans of care within state-specific guidelines. Submit accurate, complete, and timely documentation in collaboration with physician orders. Ensure patients meet home health eligibility and medical necessity guidelines as defined by payer sources. Requires current RN licensure, CPR certification, and a dependable vehicle.
