Registered Nurse (RN) - Daytime

Provide comfort-focused in-home nursing care for patients with prognoses of six months or less, including delivering basic hospice education to patients, families, and caregivers. Make 4-6 daily visits to private residences or assisted living facilities, serving as an advocate to ensure patient goals of care are communicated and prioritized by the interdisciplinary team. Collaborate weekly with social workers, physicians, chaplains, and aides at the local program office to support holistic care. Maintain a valid RN license and at least one year of hospital nursing experience.
