Job Summary
Conduct ongoing health assessments, monitor residents for physical and cognitive changes, and identify early warning signs of medical conditions. Provide person-centered support by facilitating understanding of diagnoses and treatment plans while reinforcing adherence to medications and follow-up care. Coordinate with providers, specialists, and pharmacies to ensure continuity, assist with appointment scheduling, and support discharge planning post-hospitalization. Maintain accurate electronic health records and collaborate with staff to monitor medication effectiveness. Attend weekly supervision and residence meetings while remaining current on compliance trainings. This role requires a New York RN with two years of experience in adult mental health or aging populations, based in East Patchogue with travel to Sayville.
Required Qualifications
- Associates Degree in Nursing
- Current and unrestricted Registered Nurse (RN) in the State of New York
- 2 years of experience working with adults with serious mental illness, chronic medical conditions, aging populations, or a combination of these
- Strong skills in care coordination, interdisciplinary collaboration and communication
- Ability to work independently while functioning as part of a multidisciplinary team
- Willingness and ability to travel between East Patchogue and West Sayville
