Job Summary
Coordinate home health services for patients by reviewing and approving patient information during start of care, recertification, resumption, or evaluation visits. Facilitate relationships between physicians, referral sources, patients, caregivers, and staff while ensuring all activities meet acceptable standards of practice and company procedures. Maintain a valid driver's license, automobile liability insurance, and dependable transportation to safely drive in all weather conditions. Requires RN licensure, two years of nursing experience, and one year in home health or hospice. Required Qualifications
- Must be a graduate of an approved school of professional nursing
- Must be licensed in the state in which they currently practice, or in accordance with the board of nursing rules for nurse licensure compact for the state in which they practice
- Must have at least two years of nursing experience
- Must have one year experience in home health or hospice
- Must have demonstrated knowledge and understanding of the federal, state, and local laws and regulatory guidelines that govern a home care operation
- Must have basic demonstrated technology skills, including operation of a mobile device
- Must possess a valid state driver license
- Must maintain automobile liability insurance as required by law
- Must maintain dependable transportation in good working condition
- Must be able to safely drive an automobile in all types of weather conditions
