Job Summary
Coordinate home health services to ensure patient care meets standards and company procedures. Review and approve patient information during start of care, recertification, resumption of care, or evaluation visits. Facilitate relationships between physicians, referral sources, patients, caregivers, and employees. Maintain a valid driver's license, automobile liability insurance, and dependable transportation for safe driving in all weather conditions. Requires RN licensure, two years nursing experience, one year home health or hospice experience, and knowledge of federal, state, and local regulatory guidelines.
Required Qualifications
- Graduate of an approved school of professional nursing
- Licensed in the state in which they currently practice, or in accordance with the board of nursing rules for nurse licensure compact for the state in which they practice
- At least two years of nursing experience
- One year experience in home health or hospice
- Demonstrated knowledge and understanding of the federal, state, and local laws and regulatory guidelines that govern a home care operation
- Basic demonstrated technology skills, including operation of a mobile device
- Valid state driver license
- Maintain automobile liability insurance as required by law
- Maintain dependable transportation in good working condition
- Be able to safely drive an automobile in all types of weather conditions
