Job Summary
Coordinate home health services for patients by reviewing and approving patient information during start of care, recertification, resumption, or evaluation visits. Facilitate relationships between physicians, referral sources, patients, caregivers, and staff while ensuring delivery meets acceptable standards and company procedures. Maintain a valid state driver's license, automobile liability insurance, and dependable transportation to safely drive in all weather conditions. Requires RN licensure, two years nursing experience, one year home health or hospice experience, and knowledge of federal and local regulatory guidelines.
Required Qualifications
- Graduate of an approved school of professional nursing
- Licensed in the state in which they currently practice, or in accordance with the board of nursing rules for nurse licensure compact for the state in which they practice
- At least two years of nursing experience
- One year experience in home health or hospice
- Demonstrated knowledge and understanding of the federal, state, and local laws and regulatory guidelines that govern a home care operation
- Basic demonstrated technology skills, including operation of a mobile device
- Valid state driver license
- Automobile liability insurance as required by law
- Dependable transportation in good working condition
- Ability to safely drive an automobile in all types of weather conditions
