Job Summary
Conduct thorough patient assessments, administer prescribed medications, and provide assistance with Activities of Daily Living in the home. Educate patients and families on health maintenance, monitor symptoms, and document care in the EMR system. Coordinate schedules with patients and communicate with case managers and physicians to ensure a unified approach. Offer emotional support and prepare clinical notes reflecting the patient's story. Requires a valid RN license, BLS certification, and willingness to travel. Pay range is $36.00 - $38.00 /Hr.
Required Qualifications
- A valid and active RN License in the state of practice.
- A willingness to travel, bringing care directly to patients' homes.
- Graduation from an accredited school of nursing.
- Current BLS Certification.
- Ability to pass a state & federal background check, ensuring trust and safety.
- Malpractice Insurance is required.
- Strong problem-solving and critical thinking skills, coupled with a warm, empathetic approach.
