Registered Nurse (RN)

Case manage, assess, plan, develop, and implement a comprehensive home health care plan for patients. Measure vital signs, administer IVs, change dressings, and clean wounds as needed while providing skilled nursing care, preventative rehabilitative procedures, and prescribed treatments across various complex home situations. Work closely with a supportive patient care team including physicians, therapy, social services, and home health aides to manage assigned caseloads. Provide patient and family education for disease management and health promotion as directed by the physician's plan of treatment. Maintain the most upmost standard of care. Requires New York State RN license, one year of recent hands-on experience, BLS certification, and a valid NY driver's license.
