Job Summary
Case manage, assess, plan, develop, and implement comprehensive home health care plans for patients. Measure vital signs, administer IVs, change dressings, and clean wounds as needed while providing skilled nursing care, preventative rehabilitative procedures, and prescribed treatments across various complex home situations. Work closely with physicians, therapy, social services, and home health aides to support your assigned caseload, and provide patient and family education on disease management and health promotion. Work 5 x 8-hour shifts (8:30 AM to 4:30 PM) with rotating weekends.
Required Qualifications
- New York State RN license
- Minimum of one year of recent hands on experience as a Registered Nurse
- Basic Life Support Certification
- Valid New York State Driver's License
