Registered Nurse (RN) Case Manager

Conduct comprehensive patient and family assessments to determine hospice needs, including physical evaluations and health history. Develop, implement, and revise the Plan of Care in collaboration with the Hospice Medical Director, patient, and family. Administer medications and treatments as prescribed, while educating patients and families on self-care, medications, and diet. Maintain accurate clinical documentation and coordinate care with physicians and interdisciplinary teams. Provide emotional support and counseling, arrange for necessary medical equipment, and ensure HIPAA compliance. Participate in on-call visits including evenings and weekends, attend group meetings, and supervise ancillary personnel. Requires a current Registered Nurse license, valid driver's license, and ability to lift up to 50 pounds.
