Registered Nurse (RN) Case Manager

Complete initial and ongoing comprehensive assessments to determine hospice needs and develop individualized care plans with patients and families. Administer medications and treatments as prescribed while educating caregivers on care techniques, medications, diet, and symptom management. Lead meaningful end-of-life conversations and ensure exceptional symptom management with clinical excellence. Rotate on-call duties to ensure seamless care transitions. Requires 1+ years of acute care experience, valid driver's license, and reliable transportation. Full-time 8:00am to 5:00pm schedule with benefits including health, dental, vision, 401(k) match, and tuition reimbursement.
