Registered Nurse (RN) / Case Manager

Conduct in-home assessments, develop individualized care plans, and provide ongoing clinical case management for older adults. Support caregivers by educating them on care protocols, responding to condition changes, and ensuring timely documentation. Serve as a clinical liaison for clients, families, physicians, and healthcare professionals while collaborating with the Director of Nursing to maintain care continuity. Address client concerns and coordinate clinical follow-up, partnering with community referral sources to support smooth care transitions. Requires an active Massachusetts RN license, two years of nursing experience, and a valid driver's license. Based in Bedford, MA, with competitive pay ranging from $45 to $52 per hour.
