Registered Nurse - RN Case Manager

Conduct in-home nursing visits for former Nuclear Weapons Workers with chronic or terminal illnesses, performing physical assessments, ensuring home safety, and managing medication plans. Coordinate client care conferences, evaluate clinical documentation, and review shift reports for policy adherence. Serve as a local on-site clinical resource, assist staffing coordinators with personnel scheduling, and perform supervisory visits to facilitate problem resolution. Collect and submit data on infections and performance improvement activities while assisting with marketing initiatives as requested.
