Job Summary
Conduct in-home nursing visits for clients with chronic or terminal illnesses, performing physical assessments, ensuring home safety, and managing medication planners. Coordinate care conferences, evaluate clinical documentation, and supervise caregivers while collaborating with the regional director to improve services. Perform supervisory visits, review shift reports, and assist with staffing and equipment tracking as needed. Maintain current CPR certification and adhere to agency policies. Required Qualifications
- Graduate of a state approved school of professional registered nursing
- Current, unrestricted RN license in the state(s) of practice
- Minimum of two (2) years nursing experience including one (1) year in home care or closely related field
- Current CPR certification
Desired Qualifications
- BSN preferred
- One (1) year of supervisory and/or case management experience preferred
