Job Summary
Conduct weekly in-home nursing visits for clients with chronic or terminal illnesses, performing physical assessments, ensuring home safety, and managing medication plans. Coordinate care conferences, evaluate clinical documentation, and supervise caregivers while collaborating with regional directors to improve services. Perform supervisory visits, review shift reports, and track equipment as needed. Maintain on-site availability during operating hours to facilitate problem resolution and support client needs.
Required Qualifications
- Graduate of a state approved school of professional registered nursing
- Current, unrestricted RN license in the state(s) of practice
- Minimum of two (2) years nursing experience including one (1) year in home care or closely related field
- Current CPR certification
Desired Qualifications
- BSN preferred
- One (1) year of supervisory and/or case management experience preferred
