Job Summary
Conduct in-home nursing visits weekly to perform physical assessments, ensure home safety, and manage medication plans for clients with chronic or terminal illnesses. Coordinate care by supervising skilled and non-skilled caregivers, reviewing clinical documentation, and participating in care conferences. Serve as a local clinical resource to support client needs, assist with staffing decisions, and perform supervisory visits to resolve issues. Collaborate with the regional director on performance improvement initiatives and agency marketing activities as requested.
Required Qualifications
- Graduate of a state approved school of professional registered nursing
- Current, unrestricted RN license in the state(s) of practice
- Minimum of two (2) years nursing experience including one (1) year in home care or closely related field
- One (1) year of supervisory and/or case management experience
- Current CPR certification
