Job Summary
RN Case Manager conducts in-home nursing visits for clients (former Nuclear Weapons Workers) with chronic/terminal illness, performing direct care and case management. Responsibilities include planning, implementing, and evaluating care; coordinating with the regional director and staff; participating in client care conferences; supervising and training clinical personnel; reviewing documentation for accuracy; contributing to performance improvement initiatives; and engaging in activities such as marketing visits to discuss Agency programs. The role emphasizes building one-on-one patient relationships, ensuring home safety and appropriate staffing within scope of practice, and providing on-site clinical support as needed.
Required Qualifications
- Graduate of a state approved school of professional registered nursing
- Current, unrestricted RN license in the state(s) of practice
- Minimum of two (2) years nursing experience including one (1) year in home care or closely related field
- One (1) year of supervisory and/or case management experience preferred
- Current CPR certification
