Registered Nurse (RN) Case Manager

Job Summary
Provide home-based care to patients in Northern Colorado (Thornton, Northglenn, Westminster, Broomfield) as a Home Health RN. Assess patients, perform progress documentation, educate patients and families on plan of care, medications, therapy, diet, and home safety, coordinate discharge, and oversee LPNs, CNAs, and aides. Part-time with potential to build to full-time; competitive per-visit compensation with travel time and mileage reimbursement. Requires Colorado RN license, 1200 hours of RN experience, CPR/BLS, and willingness to travel to assigned visits. Required Qualifications
- Active Registered Nurse (RN) license in Colorado
- Graduate of an accredited nursing school or program
- At least 1200 hours of RN experience
- Health care CPR/BLS certification
- Background check and drug test as required
