Job Summary
Registered Nurse (RN) Case Manager based in Laconia, NH, serving the Greater Lakes Region. Responsible for managing a defined caseload, delivering direct patient care in the community/home setting, coordinating patient care with healthcare providers, implementing care plans, maintaining accurate documentation, and adhering to company policies and regulatory requirements. Requires NH RN license, at least two years of acute care nursing experience, and BLS certification; requires reliable transportation to travel within the region. Flexible schedule with a Monday–Friday, 8:30am–5:00pm framework and potential for flexible days.
Required Qualifications
- Valid Nursing License (RN) in the State of New Hampshire
- Minimum of two years acute care nursing experience
- Current CPR BLS Certification (monthly renewal classes available and paid for by Interim HealthCare)
- Reliable transportation to travel within the Greater Lakes Region
- Home care experience is preferred but not required
