Job Summary
Provide education to patients and families regarding the care process and guide healthcare decisions. Consult with the attending physician and medical director on condition changes requiring order or plan-of-care adjustments. Coordinate hospice care by collaborating with team members and families to ensure needs are met. Conduct initial and ongoing comprehensive assessments, performing evaluations and informational visits. Work Monday through Friday in Kansas City, MO, with a salary range of $75,000 to $90,000. Requires a Missouri or Compact RN license and valid driver's license.
