Registered Nurse (RN) Case Manager - Hospice

Job Summary
Conduct initial and ongoing OASIS assessments and comprehensive physical evaluations to determine home care needs for terminal patients. Develop and revise nursing care plans incorporating therapeutic, preventative, and rehabilitative actions while administering prescribed medications and treatments. Coordinate patient care as a Case Manager by prioritizing interdisciplinary team needs, providing health education on disease processes and end-of-life care, and supervising aides and LPNs/LVNs. Prepare clinical notes for physicians every sixty days, manage equipment arrangements, and fulfill on-call duties as defined by policy.
Required Qualifications
- Current and valid nursing licensure in the state employed
- Current and valid CPR certification
- Licensed driver with an insured automobile in good working order
- Capability to perform job functions with or without accommodations
- Compliance with acceptable professional standards and practice
Desired Qualifications
- Bachelor's degree with one (1) year of home health and/or hospice care experience
