Registered Nurse (RN) Case Manager

Registered Nurse (RN) Case Manager provides clinical oversight for hospice patients in patient homes in Northern Rhode Island (Cumberland and Woonsocket). Responsibilities include conducting initial and ongoing patient assessments, collaborating with physicians to manage plans of care, regularly evaluating and updating the plan of care, delivering ongoing training and education to patients and families, initiating appropriate nursing procedures and referrals to community services, preparing point-of-care clinical documentation, and coordinating services with physicians, management, and staff. Qualifications include RN in Rhode Island (MA licensure preferred), graduate of an accredited nursing program with RN required, and 1–3 years of clinical experience; BSN preferred.
