Registered Nurse (RN) Case Manager | Homecare Rock Rapids

RN Case Manager in home health/hospice set to manage care planning, utilization of services, coordination of care, and education of the interdisciplinary team. Responsibilities include Start of Care visits, post-hospital evaluations, patient re-certifications, discharges, supervisory visits, and developing the plan of care in collaboration with the physician. Utilizes the nursing process with comprehensive assessments to recommend efficient/resourceful utilization of care, provides disease management education to patients and families, and collaborates with physicians and the care team to achieve positive outcomes. Includes travel to patient homes for care delivery and ongoing education of patients, families, and caregivers.
