Job Summary
Manage caseloads including supervision of LPN visits and coordinate services for physicians, insurance companies, and visiting nurses. Conduct thorough initial assessments, interpret diagnostics, and develop personalized care plans aligned with physician orders for conditions such as CHF, COPD, Diabetes, and wound care. Provide patient education on medication management and collaborate with Physical Therapy, Speech Therapy, and Occupational Therapy professionals. Complete charting during visits using AI-powered support to maintain work-life balance. Requires Kansas or Compact RN license and driver's license. Service areas include Overland Park, KS and surrounding counties.
