As a Home Health RN Case Manager in Jefferson City, MO, you will manage caseloads, supervise LPN visits, coordinate with physicians and therapy teams (PT, OT, Speech), educate on CHF/COPD/Diabetes, interpret diagnostic data, and develop physician-order-driven care plans while leveraging AI-powered charting to support on-site patient care and balance workload. Your responsibilities include routine visits, starts of care, and evaluations, with strong collaboration across disciplines and effective communication with health insurers to ensure coordinated services.
