Job Summary
Assess patients' physical and psychosocial needs, develop individualized care plans, and coordinate with physicians and interdisciplinary teams to achieve optimal outcomes. Establish realistic treatment goals, reconcile medications, and complete physician orders, OASIS documentation, and levels of care in accordance with VNA policies. Effectively manage assigned caseloads by establishing daily work plans, triaging visits and messages by urgency, and providing education and advocacy to patients and families. Collaborate on discharge planning and crisis interventions while maintaining compliance with regulatory standards. Full-time, 40-hour days in Malden, MA area.
Required Qualifications
- Massachusetts/New Hampshire RN Licensure
- Valid state issued Driver's License
- One (1) year of acute medical/surgical nursing experience
- Cardiopulmonary Resuscitation (CPR) Certification
- Knowledge of pharmacology and medication administration and reconciliation
- Expert working knowledge of all third party payers and regulatory bodies and agency policies on issues related to documentation and care provided

- Ability to assess, plan, implement, and evaluate the care needs of a designated group of patients
- Ability to provide nursing care and identified clinical support tasks within the scope of practice for achievement of the patient's plan of care
- Ability to assess patient/family learning needs, style and limitations and adjust for delivery of information
- Ability to establish realistic goals and develop plans of treatment in cooperation with the patient, family and members of the health care team
- Ability to collaborate with patient/family and other health care providers and/or community resources with planning of care and discharge
- Ability to complete physicians' orders, levels of care, and OASIS on all patients assigned, in accordance with patient care policies
- Ability to reconcile medications with patient and physician consistently
- Ability to demonstrate ability to cope with patient/family emotional stress and provide appropriate supportive care
