Job Summary
Support patient recovery and ability to stay at home by educating families on disease management and providing clinical oversight to the interdisciplinary team. Case manage patients in the Greater Tulsa area while empowering them to reach healthcare goals. Requires an RN license, two years of nursing experience, home health experience preferred, current driver's license, and CPR/BCLS certification. Full-time or part-time scheduling available with flexible hours.
