Registered Nurse (RN) Case Manager

Job Summary
Develop care plans for designated patients under physician direction by monitoring assessments, patient interactions, and medical records to ensure progress toward desired outcomes. Identify and resolve barriers to timely discharge while advocating for patients and collaborating with social work teams on discharge planning. Complete physical, mental, and psychosocial assessments within 24 hours of admission, communicate with multidisciplinary teams, and document all interventions. Assist patients and families with coping skills, discuss financial alternatives, and serve as a liaison between Froedtert South and external referral sources. Lead interdisciplinary meetings as a resource person and participate in educational programs to maintain professional expertise.
Required Qualifications
- RN Wisconsin license
- 3-5 years of hospital clinical experience as a registered nurse
- Proven ability to function effectively with others to develop and implement a plan to address the needs of the patient during hospitalization and after discharge
- Licensure to practice as a Registered Nurse in the State of Wisconsin
- Nursing experience in the healthcare arena with previous clinical experience
- Must be able to read, write, speak, hear and comprehend the English language
