Job Summary
RN Case Manager in Texas for Adaptive Home Health. Field-based role delivering high-acuity home health care, owning patient assessments and skilled visits including OASIS, with focus on wound care and infusion therapy. Build and maintain a care plan through ongoing communication with physicians, care team, patients, and families; drive case management activities like timely follow-ups and barrier resolution to keep patients progressing toward goals. Administer medications and treatments as ordered; coordinate services (PT/OT/ST/MSW/HHA); supervise LVNs and HHAs as applicable; participate in case conferences and trainings to align on quality, safety, and clinical outcomes. Leverage charting tools and scheduling product to support efficient visits; schedule flexibility offered with field-based autonomy and a supportive clinical/operational team.
Required Qualifications
- Current Texas RN license
- 3+ years of clinical nursing experience
- Comfort with wound care and infusions
- CPR required (BLS)
- Current Texas driver's license and comfort with field-based work
