Registered Nurse (RN) - Case Manager

Registered Nurse (RN) Case Manager in field-based adaptive home health in Aurora, CO. You will own patient assessments and skilled visit documentation including OASIS, deliver high-acuity home health care with a focus on wound care and infusion therapy, build and maintain a clear plan of care through communication with physicians and families, advocate for patients, drive case management responsibilities including care coordination and timely follow-ups, coordinate services to ensure the plan of care is executed, administer medications and provide teaching, and supervise LVNs and HHAs where applicable.
