Registered Nurse (RN) - Case Manager

Own patient assessments and skilled visit documentation including OASIS, delivering high-acuity home health care with a focus on wound care and infusion therapy. Build and maintain a clear plan of care through consistent communication with physicians, care team members, patients, and families. Serve as the patient advocate, ensuring continuity of care across disciplines while driving case management responsibilities including care coordination, timely follow-ups, and barrier resolution. Administer medications and treatments as ordered, provide patient and caregiver teaching, and coordinate services with PT/OT/ST/MSW/HHA teams. Provide supervision and guidance to LVNs and HHAs when applicable, and participate in case conferences to align on quality, safety, and clinical outcomes. Full-time Monday–Friday schedule with some weekends as needed.
