Job Summary
RN Case Manager delivering skilled home health care in Texas field-based, focusing on wound care and infusion therapy, coordinating care across physicians, patients, and families, and ensuring a clear plan of care with consistent communication. Responsibilities include patient assessments and skilled visit documentation (including OASIS), administering medications and treatments, supervising LVNs and HHAs when applicable, driving case management through follow-ups and barrier resolution, coordinating with PT/OT/ST/MSW/HHA, and contributing to quality and safety through case conferences and ongoing education; working with the team to reduce administrative friction and leverage technology to streamline visits and scheduling.
Required Qualifications
- Current Texas RN license
- 3+ years of clinical nursing experience
- Comfort with wound care and infusions
- CPR required (BLS)
- Current Texas drivers license and comfort with field-based work
