Registered Nurse (RN) - Case Manager

Registered Nurse (RN) - Case Manager responsible for delivering in-home, high-quality home health care in Detroit with a focus on wound care, comprehensive in-home assessments (OASIS, SOC/recert/ROC, discharge), and building/overseeing the plan of care with physicians while supervising LPNs and HHAs. The role emphasizes strong referral-source relationships, charting discipline, and a scalable pod model to achieve a full caseload (~30 productivity points/week) with a quality standard that reduces avoidable hospitalizations. The clinician will be the face of Adaptive to patients, families, and referral partners, leverage software to accelerate charting, and participate in team-based meetings (case conferences, huddles).
