Responsibilities include direct care and case management, coordinating with regional leadership, planning and evaluating care, participating in client care conferences, ensuring compliance with scope of practice, staffing coordination, clinical documentation accuracy, performance improvement activities, and participation in marketing activities as requested. Schedule is self-directed with emphasis on one-on-one patient relationships and ability to balance care management with personal commitments.
